By submitting this application, I/we authorize Mineral Tech Holdings to make inquiries into the banking and business/trade references provided above. I/we also authorize Mineral Tech Holdings to obtain a business credit report and, for any sole proprietorship or partnership, a personal credit report for the purpose of evaluating this application for credit.
